Supplemental Sick Leave Bank program enrollment open until Nov. 29

In the event an employee experiences a catastrophic illness or event that requires a long absence from work, the Supplemental Sick Leave Bank (SSLB) program can provide some help.

The SSLB program provides participants an opportunity to request up to 30 additional leave days per year in the event of a catastrophic illness requiring a physician-approved absence from work exceeding seven (7) workdays. The employee must exhaust all earned and advanced vacation, state, and local personal leave to be eligible for SSLB days. Additional program information is available here

This is the final opportunity to enroll for the 2016-2017 program year, and the enrollment deadline is Tuesday, Nov. 29, 2016.

To enroll, simply complete the SSLB 2016-2017 Membership Enrollment Form and have available one local personal leave day to contribute to the bank for the 2016-17 program year (Sept. 1, 2016 through Aug. 31, 2017).

Program membership will begin effective Nov. 7, 2016 for this enrollment period. The qualifying local leave day must be available and will be deducted within one or two pay cycles following Nov. 29. It is the participant’s responsibility to verify the contributed day has been deducted as a negative leave adjustment on his or her pay statement. Employees who do not have an available local personal leave on the date the deduction is made in SAP will be ineligible to participate this school year (even if a leave day was available when the Membership Form was submitted).

Submit completed forms to Employee Support Services, Williams Middle School – Annex Building, 6100 Knox St., Houston, TX 77091, or via interoffice mail – Route 8, or fax to (713) 695-5723. Faxed submissions are recommended to ensure receipt by the enrollment deadline.
