Back-to-school parent packets

Download and print up-to-date compliance forms and useful one-pagers

Members of Team HISD preparing parent packets for the start of the 2017-2018 school year can download all compliance documents at www.houstonisd.org/parentforms. The website contains the most up-to-date forms HISD Is required by law to distribute to parents and guardians along with useful one-pagers on topics such as proper online behavior, the importance of attendance, and the district’s electronic services acceptable use policy.

The documents should be downloaded and printed by each campus, except for items listed below. All schools should have already received a campus supply of the following items:

  • 2017-2018 Code of Conduct acknowledgment form/request for hard copy
  • 2017-2018 student media release form
  • 2017-2018 school year calendar
  • SchoolMessenger “How do you want to be communicated with?” parent flier

If your school has not yet received the campus supply of these documents, please call the HISD Warehouse at 713-676-9292.

Code of Conduct

All schools will receive a 10 percent supply of the 2017-2018 Code of Conduct in late August. The supply should be distributed ONLY to those students/families who request a hard copy. The 2017-2018 code is currently available online to all staff, students, and parents in four languages at www.houstonisd.org/codeofconduct . It also will be posted to all school websites.

Free and Reduced Lunch form

All schools will receive a campus supply of the National School Lunch Program application/income survey form in late August. In addition to the printed form, the meal application/income survey is also available online in English, Spanish, Vietnamese, and Arabic. Nutrition Services encourages all principals to direct parents to http://mealapps.houstonisd.org to submit a meal application/CEP survey for their student online before school starts.
